The best guide to finding Medicaid consulting firms.

Starting by identifying different introduction functions and their relationships to each other, several techniques for further connecting content-similar functions and binding them together will then be presented.

Home health care

Home health care services for older adults are an integral component of their continuum of care, offering services like physical therapy, home health aides, and personal care to keep seniors living independently in their own homes and maintaining independence. Although home health care can be costly, Medicare helps cover some of its expenses.

Specific criteria must be fulfilled for home health care to be effective. You should generally be homebound after being discharged from a hospital or rehabilitation center. Then, your physician must certify your need for short-term skilled home health care through Medicare-approved agencies; additionally, every 60 days, they must sign off on the certification of this need by signing the Home Health

Certification form.

Certified Home Health Agencies (CHHAs) are among the most prevalent home health providers under Medicare Part A. They typically offer skilled nursing and therapy services as well as home health aides. CHHAs must also be licensed by New York State’s Department of Health; additionally, they are required to use an assessment tool known as the Outcome and Assessment Information Set (OASIS), which collects and reports on data about effectiveness. CMS reports subsets of this OASIS data via its Care Compare website.

Home health care under Original Medicare and Medicare Advantage plans can be similar, although with some differences. Medicare Advantage plans tend to forgo nonmedical home services like meal delivery or homemaker services, while Original Medicare only pays out home health benefits when medically necessary and reasonable.

Media service offers long-term care and assisted living solutions for individuals in New York, as well as home healthcare services. Its team of highly trained professionals includes nurses, physical therapists, occupational therapists, and speech therapists, as well as social workers, chaplains, and bereavement counselors.

Home Healthcare Solutions provides affordable home healthcare services, including medication management and therapy, for its clients. Its aim is to deliver top-quality care while keeping costs under control by offering multiple payment options, including Medicare Advantage Plans and private insurance policies.

Medication management

Medication management services provide integrated healthcare services designed to assist patients in managing their medications effectively and safely. They differ from disease state management and patient counseling as they specifically target specific medications and their use. They aim to increase adherence to prescribed drugs while decreasing adverse drug events and inappropriate usage and reduce overall healthcare costs while simultaneously enhancing patient safety.

Medication Management Services are provided as part of the Medicare Part D Medication Therapy Management Program and are provided free to members. The Medicare Prescription Drug Improvement and Modernization Act of 2003 mandated that Part D Plans offer Medication Therapy Management services to their members who meet specific criteria; for the first time, Congress explicitly mentioned pharmacists – a fantastic achievement in itself for our profession!

Pharmacists are accountable for ensuring the safe and effective use of medications within the community. To ensure patients get optimal results from their medicines, pharmacists must conduct medication reviews and offer education services for patients taking medication. Medication management services also play a vital role in helping identify potential issues while proposing changes that improve the quality of life for all involved parties.

Express Scripts, a Cardinal Health company, contracts with Medica to offer Medication Management Services to members eligible for this program. These services include Comprehensive Medication Reviews and Targeted Medication Reviews. Medica members eligible for MTM will receive an invitation letter informing them they have been automatically enrolled, including information and instructions on opting-out procedures. For more information or assistance, please call Medica member services using their ID card number or TTY 711.

The Medicare Part D Medication Therapy Management (MTM) program offers one-on-one reviews to make sure you’re receiving maximum benefit from your medications. MTM pharmacists will review any Medicare Part D covered health conditions you take medicine for, such as arthritis, osteoporosis, diabetes, high blood pressure, or heart failure – any issues with them during a CMR or TMR review will be brought up with either you or your physician immediately.

Nutritional counseling

Nutritional counseling services can be invaluable for enhancing overall health and quality of life by supporting diet changes. Registered dietitians or other qualified healthcare professionals offer these services. Medicare Part B usually covers these visits; check with your insurance provider if any additional coverage exists for nutritional counseling sessions.

Weight loss and diet changes can be essential in managing the symptoms of chronic diseases such as diabetes, asthma, and COPD. Dietary changes may even reverse the progression or prevent flare-ups of these conditions; nutritional counseling provides support for making gradual and manageable dietary modifications over time.

Many Medicare Advantage (MA) Plans include coverage for nutritional therapy as part of their preventive care benefits; however, coverage varies depending on which plan you select and whether or not it offers in-network or out-of-network options. Nutritional therapy is usually provided by registered dietitians and registered dietician nutritionists (RD/RDN). Nutritional therapy offers one-on-one assistance to help individuals establish healthier eating habits and make informed choices. Ueshima et al. 2023 conducted an in-depth analysis of randomized controlled trials assessing nutritional counseling as part of anticancer therapy in patients who had incurable advanced or recurrent cancer that was resistant to current anticancer therapies and who required immunosuppression therapy. Their analysis indicated that including nutritional counseling within SMS telemedicine programs proved at least as effective as traditional telephone or video-conference counseling sessions for providing nutrition advice.

Personal care

Personal care services are integral for maintaining independence and improving well-being. They assist with basic daily tasks that individuals may find themselves unable to perform themselves—bathing, dressing, grooming, eating, and toileting are among them—and are sometimes known as Instrumental Activities of Daily Living (IADLs). Medicare usually covers personal care services deemed medically necessary for an individual patient residing at home, in their community, or in skilled nursing facilities.

Individuals in need of personal care services should create a plan of care in collaboration with healthcare professionals such as doctors or nurses. This will streamline the process and ensure all paperwork is submitted in an organized fashion, which may also ensure coverage by Medicare.

Patients must understand the differences between personal care services and home health services. While both can play an essential part in improving health, home health services typically involve medical treatments provided by licensed nurses, whereas Medicare only covers them when medically necessary to treat illness or injury.

Medicare-eligible individuals in need of personal care services can access them through private payment, specific health insurance plans, and Medicaid-managed care organizations. Alternatively, local departments of social service offer programs designed to allow individuals to remain living within their communities rather than opting for assisted living or nursing home facilities.

OIG reviews conducted previously have identified States as experiencing difficulty complying with PCS requirements, such as providing adequate oversight of providers and avoiding improper billings and payments. This review will assess whether improvements have been made in PCS oversight and monitoring and whether its safeguards support medical necessity, quality, and safety.
